Canned Heat - Amphetamine Annie - Sounds like a really poor recording; like it was ripped straight from a vinyl record by a cassette recorder held up to the speaker.
Tool - Schism sounds like crap, too.

There's a song called "08 My Sister Rose" which doesn't follow the artist-title requirement.

...and why does every single Dragonforce song sound like "Through the Fire and the Flames" with different lyrics?

I don't know who it really is (Maybe Richard Cheese), but the "Wierd 'Al' Yankovic" song "I Like Small Butts" is not Wierd "Al" Yankovic.
